All Singapore residents should be vaccinated: expert committee

SINGAPORE — The Pfizer-BioNTech COVID-19 vaccine is suitable for use in those aged 16 and above, and everyone residing in Singapore who is medically eligible for vaccination against COVID-19 should be vaccinated.

These were part of the recommendations submitted to the government by the Expert Committee on COVID-19 Vaccination, said the Ministry of Health (MOH) in a news release on Sunday (27 November).

The committee also advised that the government prioritise the use of the vaccine on two groups: those at high risk of contracting the coronavirus – such as healthcare and other frontline workers – and those who are most vulnerable to developing complications from it – such as the elderly and those with “vascular medical co-morbidities”.

Chaired by Associate Professor Benjamin Ong, who is also senior advisor to MOH’s director of medical services, the committee also noted that vaccination “should remain voluntary”.

The group was convened in October to advise the government on Singapore’s COVID-19 vaccination strategy. It submitted its recommendations to the government on Thursday.

“The Expert Committee has independently reviewed the clinical data on the safety and efficacy of the Pfizer-BioNTech COVID-19 vaccine, and concurs with the indications for the interim authorisation granted by the Health Sciences Authority for use of the vaccine in Singapore under the Pandemic Special Access Route,” said the release.

It noted that vaccine has demonstrated a high efficacy rate of 95 per cent and that the vaccine’s safety profile was consistent with the high standards set for other registered vaccines used for other diseases. Singapore received its first shipment of the Pfizer-BioNTech COVID-19 vaccine on 21 December, with more expected to arrive in the coming months.

On the matter of vaccinating the resident population – including citizens, permanent residents and long-term pass holders – the committee recommended that the government work to “achieve as high a level of population coverage for COVID-19 vaccination as possible”. This would not only reduce the size of the population at vulnerable to the coronavirus, but would also indirectly protect those who may not yet be suitable for vaccination.

As for the elderly, the committee recommended vaccinating those aged 70 and above first before moving on to those aged 60 to 69.

The committee also pointed out the importance of vaccinating “specific groups of persons who are of critical importance to the functioning of Singapore”, such as those involved in the country’s water and utilities as well as other nationally essential services.

“The prioritisation of these groups should be balanced carefully against the needs of persons prioritised based on public health grounds.

“On balance, the Committee recommends that around 5 per cent of available vaccine stocks at any given point in time should be set aside for such groups, to ensure that Singapore would be able to continue to function effectively as a nation, even in a local outbreak situation,” said the release.

“While Singapore currently has a low rate of local transmission of COVID- 19, we remain vulnerable to the threat of a surge in cases. As such, it is important that we achieve as comprehensive a coverage of COVID-19 vaccination as possible across the entire population,” said Assoc Prof Ong in the release.

Moderna COVID-19 vaccine may cause side effects for those with cosmetic facial fillers: FDA

https://nypost.com/2020/12/25/moderna-covid-vaccine-has-caused-side-effects-for-those-with-cosmetic-facial-fillers/

People with cosmetic facial fillers could experience swelling and inflammation with one of the coronavirus vaccines, the FDA advisory committee noted.

According to the committee, several trial participants with fillers have already experienced side effects. A California-based dermatologist said the reaction was immunological, ABC7 reported on Thursday.

“Your immune system which causes inflammation is revved up when you get a vaccine, that’s how it’s supposed to work,” said Dr. Shirley Chi, who noted the side effects were easily treated by medical personnel.

“So it makes sense that you would see an immune response in certain areas where they see some substance that is not a naturally occurring substance in your body.”

She said, however, that the side effects shouldn’t stop people from obtaining the vaccine.

“In these cases the patients all had swelling and inflammation in the area that was given the filler,” Chi said.

“A couple of the patients had cheek filler six months prior to their vaccine and one patient had lip filler done two days after the vaccine. All were treated with steroids and anti-histamines and all of their reactions resolved.

Moderna’s COVID-19 vaccine received approval from the FDA on Friday, offering an alternative to one from Pfizer and BioNTech.

The two work “better than we almost dared to hope,” NIH Director Dr. Francis Collins told The Associated Press. “Science is working here, science has done something amazing.”

Early results of large, still unfinished studies show both vaccines appear safe and strongly protective although Moderna’s is easier to handle since it doesn’t need to be stored at ultra-frozen temperatures.

Swiss person dies after COVID-19 vaccination: Officials

https://www.channelnewsasia.com/news/world/covid-19-vaccine-pfizer-swiss-dies-coronavirus-13866818

ZURICH: A Swiss person who was among the first in the country to be vaccinated with a COVID-19 shot later died, officials in the canton of Lucerne said on Wednesday (Dec 30), though they did not indicate whether the death was related to the inoculation.

"We are aware of the case," a spokesperson said, adding the matter had been referred to Swiss drugs regulator Swissmedic.

The canton did not release details including the time that elapsed between when the person received the shot and when the death occurred.

Lucerne was the site of the first vaccinations in Switzerland beginning last week, with a shot from Pfizer and its German partner BioNTech given primarily to elderly people. Switzerland has received 107,000 vaccine doses, so far, and expects to get 250,000 per month starting next year.

Swissmedic could not immediately be reached for comment.

An email and phone call to Pfizer were not immediately returned.

Pfizer and BioNTech's vaccine is the only vaccine approved, so far, in Switzerland. It was approved on an emergency basis in the United States and Britain, and has won conditional marketing approval in the European Union after clinical trials in tens of thousands of people in which no significant safety issues emerged.

Millions of doses of the vaccine have been administered by now around the world. Several people have suffered allergic reactions following shots from Pfizer-BioNTech as well as from Moderna, though those incidents were resolved quickly.

What we do and don't know about new Covid-19 mutations

Quote
Jan 5, 2021, 1:31 am SGT

PARIS (AFP) - The emergence in Britain and South Africa of two new variants of Sars-CoV-2, which are potentially far more infectious versions of the virus, has prompted widespread concern. Here is what we know - and what we don't - about the mutations.

What are they?

All viruses mutate when they replicate in order to adapt to their environment.

Scientists have tracked multiple mutations of Sars-CoV-2, the virus that causes Covid-19, since it appeared in China in late 2019.

The vast majority of mutations did not materially alter either the virus's virulence or transmissibility.

However, one mutation - variant B117, which likely emerged in southeastern England in September, according to Imperial College London - has now been detected in countries across the world, including the US, France and India.

Another variant, 501.V2, was detected in South Africa in October, and has since spread to several nations, including Britain and France.

Both have multiple mutations to the virus, most notably on its spike protein - the part of the virus that latches on to human cells and helps it spread.

Specifically, the mutated versions have an altered receptor binding domain known as N501Y, which is situated on the virus' protein spike and which allows easier access to the ACE2 receptor in human cells.

This makes the mutated versions potentially more infectious than other strains.

The European Centre for Disease Control says that while there is "no clear relationship" between enhanced ACE2 binding and increased transmissibility, "it is plausible that such a relationship exists".

(Read the whole article from the link)

Will vaccines still work?

As vaccination campaigns get under way across the world, is there any reason to fear that the new mutations may not respond to the host of vaccines already on the market?

After all, the messenger RNA vaccines developed by Pfizer and Moderna trick the body into reproducing the virus's spike protein - the precise part of the pathogen that has mutated in the new versions.

The ECDC said it was too soon to know if the mutations will impact vaccine efficacy.

Last week, Henry Walke from the American Centres for Disease Control told reporters that "experts believe our current vaccines will be effective against these strains".

On Monday, however, Francois Balloux, professor of Computational Systems Biology and director at University College London's Genetics Institute said that the South African variant's spike protein mutation "helps the virus to bypass immune protection provided by prior infection or vaccination".

German vaccine developer BioNTech has said that if needed it could develop a new vaccine that would work on mutated versions within six weeks.

In a vote of confidence for the experts in Singapore who have approved the Pfizer-BioNTech vaccine for pandemic use,  will be getting vaccinated early. This is to show you, especially seniors like me, that we believe the vaccines are safe.

im still waiting
